**Please fill out this Health History Form and Insurance Verification Form before your appointment (at least to check your insurance benefits, please ^_^). Wear comfortable clothing, and bring any imaging or other health information you would like me to review. When you show up, the door may be locked, but I will be with you shortly. Contact me with any other questions!
Mon 10-6:30 (last appointment ends around 6:30), Tuesday 10-6:30, Friday 10-6:30.
Please contact me with any scheduling requests.
If you cancel within a 24-hour period, you will be charged a $50 late cancellation fee. I understand that emergencies happen, but when you cancel it is discourteous to other patients who might benefit and it is difficult to refill your time slot. If you miss your appointment, this is considered as a "no-show" and you will be charged the full amount for the appointment. Please be respectful of my time and courteous to other patients time by cancelling before the 24-hour window of your appointment to avoid these fees - thank you!
